tl;dr: complete the game on world tier 15 without dying once, while auto-increase WT is turned on.

Rules:

• start with a fresh character
• no use of existing gear or resources (no mule)
• no use of Hell’s Rangers DLC items
• no repeat of missions / no use of the story point selector
• either completed solo, duo or trio
• auto-increase World Tier must be turned on
• dying adds 10 minutes to the overall time

The run ends, when you enter the Caravel bridge on WT 15.
