The Mod team has been discussing making changes similar to what you've proposed, most likely being the addition of a new category for a more true "Any%" that won't require all three pieces of evidence (Or two in Nightmare mode).
As another mod, Dubzyhs, said on our Discord a couple of days ago, "We've been working hard on figuring out the logistics behind it since we want it to be a fair experience for all runners and create a ruleset where there is consistency between runs. We are not 100% guaranteed to add this category, but if it were to be added it would be around the time of the next update."
The goal would be to allow runners to have an opportunity to use game knowledge and potentially deduction (Particularly in Standard runs -- those being Professional, Intermediate, and Amateur) to correctly guess the ghost, and for the recording of the run to clearly show how they came to the conclusion of which ghost they are dealing with so it's evident that they aren't just throwing darts at the journal in hopes that they guess correctly.

If you die for an Individual Level in the Perfect Game category, no, it would not be valid. You must leave via the truck.
If you die in a Full Game run (Small Maps/Launch Maps/All Maps), you may re-run the map that you died on, however the time from the first attempt will still count towards the final time. As ryl1k said, if you had a High Priestess to revive you, that would be a rare exception.
As for Nightmare: No, you're only given two evidence (Three for The Mimic) to work with. Generally, the ghost is found based on it's behaviour in Nightmare difficulty -- For example watching a flashlight while hiding during a hunt to test for Myling, seeing if there's feet to rule out a Wraith, if it turns off the breaker to eliminate Jinn, hearing slow Revenant speeds, looping a Deogen, multiple objects thrown for Poltergeist, a Phantom disappearing from a photo, etc. -- Please note that both pieces of evidence available must still be witnessed in Nightmare difficulty (And of course, all three pieces of evidence in Standard).

No, as stated in the rules: "For all runs (except for extension runs): The person recording is required to select the correct ghost type in the journal. All evidences is required to be seen on the recording(s). You are not allowed to load inside a map and guess what ghost you are dealing with then leave straight away."
