If you want to play only one mission area (f.e. Flor De Oro) than there is no cutscene and no start point.
If you want to play any% (all areas, all missions) than the next question is do you want to play NG or NG+ if NG+ than you dont have anymore a cutscene and maybe this can confuse with startpoint pick. At NG we can edit the rules and say start at rally point after cutscene.

I will do that.
I cant say you the time because i dont have played all missions and all areas. For sure i unlock everything in the single areas so idk how much time does it take to do only the missions. I went in a 5 skull area yesterday with a mate and if i calculate the time only for this area it take 1h-2h only to do the main missions because they are realy hard.
The second part is the time depends on the skill progress. You get more bullet resistance and so on. That can lower the time. I think I am done with all areas in 1 week than i can do a try for any% but it will be 10h or so because die every time xD
At this weekend i want to upload some single area runs. The first is comming up in 5 min. :P
