Lunar Dragon, All 102 exits(including glitched ones) twice, once before and after beating superstar road.

You may need to do an additional "All Star Block 1up" and "22 Star" for a true 100%. 
And maybe some hidden 1up mushrooms would be included... but since 100% completion is a vague standard in this game, I don't think this suggestion is a good idea!
