I think that categories should be categorized by difficulty level.

(Easy/Normal) and (Madhouse)

Because Madhouse difficulty has different item and enemy location. Even the position of the key item (ex:crowd key)

I thought this too, was kinda confused as to why it wasn't. I mean why would anyone pick Madhouse over Easy if it isn't in a different category?

Well each difficulty "is" a category. For the moment it's all listed together but obviously once the people starts doing NG Madhouse it's going to be separated.

Remember that the game just released and the leaderboard can't look perfect yet :)
