Changes to IL timing rules

IL timing now starts upon level fade in, rather than upon pressing "Continue" at the main menu. The rules for when timing ends for each level will remain the same.

This change was made for a variety of reasons, it removes any loading time from IL runs, it allows ILs to be submitted from full game runs, and it keeps the rules consistent with the original Doomori leaderboard's rules.

All runs affected by this change have already been retimed and updated.

For the sake of clarity, "level fade in" is considered to be the first frame the level is visible.
