we were running through the hill level on 3p, and two of us were attempting a 1-cycle, while another person stayed back. both of us (the people going for 1c) died, but i died on the loading zone, while the third person just stayed in the closet. however, it still triggered the loading zone and allowed us to skip the level. would that be considered a major glitch?

On normal I don't think there would be any issues with this, but on nightmare It might be against the rules as the blue screen (That's totally hardware dependent) did show up.
No Major Glitches rules were updated to include all disallowed glitches instead of all allowed glitches. Hopefully that makes this category less confusing. Also running with TVs in Level 188 Expanded is allowed with these new rules.
