rules could be as followed

• must be on [Gamemode]
• no hacks allowed in runs
• no crashing servers
• cannot ask to be banned
• there must be a reason the admin banned you
• link the server rules etc

Never. Encouraging banning would encourage annoying others and toxicity should not be put on a pedestal. Besides that, categories that depend on other, random people are almost never made due to being too random anyway, for example in CSGO there is no category for "fastest competitive match" for the same reason.
