Before you start a run, use the level select to go to Small Intestines. Play normally until you need to use the minecart with the big button. Push the minecart onto the button, and then walk left until the camera zooms in, but stop before the first set of drop-down spikes. Exit to the game menu. You can now start a run normally. When you get to the small intestines, superjump to the button that you normally reach by standing on the mine cart. Press the button and continue as if you have the minecart. You will need to superjump to release the trash boulder. Continue until you get to the button that lets you get the minecart normally. Press the button then walk to where previously the long platform had fallen. Save and quit to the menu, then load the game. The minecart should appear.
