A category extension board isn't necessary, in my opinion. The game isn't popular enough to warrant one anyway.
However, I disagree with the notion that a score category doesn't have merit because of some generalized "we don't do scores here" argument. While Any % is a timeless classic, speedrunning goals are arbitrary. In the abstract, we're here to complete a goal as fast as possible, and I'm sure there are hundreds, if not thousands of goals across the site that don't involve beating a game. Just a few examples: throw Debbie off a cliff in Friday the 13th, collect mirror shield in A Link to the Past, marriage % in Skyrim. Denying a goal solely based on the fact that it's score related just comes across as anti fun.
I didn't mention it in the original post, but we need rules regarding what frames the run starts and ends on so people aren't arbitrarily frame counting their runs differently.
I guess I would propose first frame of fade out on main menu after selecting difficulty and first frame of fade in on the happy birthday end screen.
I don't think save states should be used at any point during an official speedrun. The only way to actually police that would be including the hard reset in every submitted video, I guess. Jackie Chan's Action Kung Fu for NES has a similar situation where you can skip the intro off a soft reset, but the folks over there decided skipping it isn't allowed for whatever reason.
I'm alright with allowing or not allowing the skip, but I think a decision should be made so people aren't wasting their time on invalid runs or runs that are slower than they could be.

I'm in favor of whatever's the least tedious for starting new runs. In this case I guess that would mean allowing it.
I noticed this as well. Anecdotally off a power cycle you can't skip it, but it seems like you can speed through the following text with the girl and Grimace where you can't on subsequent runs. It's a bit strange.
There's a new Game Boy Color game that was made I guess in partnership with McD's called Grimace's Birthday. You can play it in your browser at https://www.grimacesbirthday.com or download the actual ROM file from archive.org and play it in an emulator of your choice. I did a couple speedruns of it and it seems fun. You can beat it in under 5 minutes. It'd be cool to see a category for it here on the site.
