Hey all, my friend and I have been avid fans of this game for years, so we decided to try our hand at speedrunning it. (Of course, SGDQ brings in a wave of new plebs).
My question is pointed towards New Game Coop, rules, and the current WR holder. The rules state empty inventories, but the current WR videos show the players starting with a pistol and a first aid spray. Is this an arbitrary item given at the beginning of the game, or am I missing something completely?

When you start a NG speedrun, you can hit 'New Game' Or chapter select. They both start out with zero gold and the same items (Handgun and FAS) By hitting chapter select, you save the time of grabbing your already available guns.
I'm pretty sure you don't start out with a First Aid in first play (No saves), simply the M92F pistol. But I could be mistaken. It's been a long time since I've done a complete restart.
I restarted on Normal difficulty and had nothing in my inventory... could be amateur difficulty.
If you are playing coop, even from a completely fresh file, like snow said you will both have the Beretta and the FAS. You equip them before you both ready up because the cutscene trigger in the butchery is when both players have drawn their guns. So instead of having to pick them up you just hit your hotkey instead to save a couple seconds. There is no way to start a coop run without accessing the inventory screen because the second player has to join and this throws you straight into that screen. So I don't see any reason why you shouldn't be able to use items that you are given automatically. When me and my buddy used to run this, we disconnected our games from the steam cloud so that it would never save the game, so we were essentially starting from no save file every run. It doesn't really matter if you start with an empty inventory or not, it only loses 2 seconds and if you play on amateur you really shouldn't even come close to dying at any point in the run no matter how bad things go.
I was actually hoping you'd respond to clarify this, because me and my buddy both actually loaded instead of turning off steam cloud. When you disabled the cloud, it also allowed for an actual "new game", right?
I saw a few forum posts that suggested to go into the Program Files and navigate to your specific save file as a means of truly starting a new game, but if disabling steam cloud works just as deleting that remote file does, then I should definitely try that.
Because when we loaded, we had inventory items we sold instead. So I could be gravely mistaken on what I considered a new game and what is truly a new game.
@KillChurch, when you disconnect from the steam cloud the game cannot write data to your save, but it can still read the local save file, meaning if you had a save before you disconnected from the steam cloud it will read that instead. You still have the new game option as far as I remember, but it isn't actually new game in the terminology we use, it's new game+ since you have all the items from your save file.
We had to manually navigate to
C:\Program Files (x86)\Steam\userdata"your user id here"\21690\remote
and rename the save file (in case I wanted to use it later for NG+). We would start every run with Leomire, who was hosting, hitting New Game and I would connect to him.
Thanks precisely what I thought had to happen. And that weird error "unable to load or save" came up.
